Patient receives prescriptions to weight loss medication that patient and nurse practitioner decide to use for best results for patient, appointments with nurse practitioners every two weeks via telemed, free access to app for tracking/logging food, dietary guidance based off of tracking/logging food in app, exercise guidance, meal prep 101, recipes and weight loss medication refills.

Once a month appointment with nurse practitioner either in person or telemed to review and/or make adjustments to current treatment plan, medication refills, continue support with diet and exercise.

* Medication that are prescribed are Phentermine, Qysmia, Contrave, Xenical, Semaglutide (Wegovy/Ozempic), Tirzepatide (Mounjaro).

Initial appointment with nurse practitioner to discuss symptoms, treatment plan options and discussion of any diagnostic test needed such as labs. Labs cost can be billed to health insurance provider or patient can pay $100 for full hormone panel through the clinic. Follow up to discuss diagnostic results and treatment plan options is included in the initial visit fee.

Includes all office visits with nurse practitioners, labs every 3 months, cost of Testosterone and supplies for home injections. Patient can come into clinic for weekly injections and the clinic does provide education on self injections or teaching support person how to administer home injections.

*TRT Monthly is membership based with automatic draft payments once a month. May cancel membership/autodraft 90 days from the date of testosterone prescription. please allow up to 30 days to have draft discontinued.
