$100 per month for months 3-6
Once a month appointment with nurse practitioner in person or telemed to review and/or make adjustments to current treatment plan, medication refills, continue support with diet and exercise.
$50 per month after 6 months
Appointment with nurse practitioner every 2-3 months in person or telemed to review and/or make adjustments to current treatment plan, medication refills, continue support with diet and exercise.
